Mount Sinai to Begin the Transfer of COVID-19 Antibodies into Critically Ill Patients

The Mount Sinai Health System this week plans to initiate a procedure known as plasmapheresis, where the antibodies from patients who have recovered from COVID-19 will be transferred into critically ill patients with the disease, with the expectation that the antibodies will neutralize it.
The process of using antibody-rich plasma from COVID-19 patients to help others was used successfully in China, according to a state-owned organization, which reported that some patients improved within 24 hours, with reduced inflammation and viral loads, and better oxygen levels in the blood.

Positive updates about COVID-19

If you’re like me, you’ve probably been spending a lot of time watching the news these days.
And some of that news is pretty scary.
​
But it’s not ALL scary.
I want you to realize that.
As we collectively wrap our heads around the enormity of the situation we’ll be facing for the next few weeks or months, it’s vitally important for us to remember that:
There’s a lot of good news out there, too.
Communities are coming together like never before. Neighbors are helping each other and making sure the vulnerable individuals in their midst are being well taken care of. People are sharing moments of profound gratitude and connectedness.
It’s a beautiful thing to see.
So if you find yourself being overwhelmed by all of the negative news, look for the good!
There have been many positive developments in the past week that offer hope and proof that we will survive this experience and come through it stronger than ever.

Here are just a few of the stories I’ve seen lately:
  • China closed down its last coronavirus hospital this week because the number of new cases has fallen so low
  • Of 80,000 people sick from COVID-19 in China, more than 70% have recovered and been discharged from hospitals
  • Doctors in India, France, and the US have been successfully treating COVID-19 with a combination of anti-malarial and other drugs
  • Researchers of the Erasmus Medical Center in Rotterdam claim to have found an antibody against COVID-19
  • ​A 103-year-old Chinese grandmother has made a full recovery from COVID-19 after being treated for 6 days in Wuhan, China
  • Apple has reopened all 42 of its stores in China
  • Scientists in China have figured out how COVID-19 breaks into human cells, which is critical information for creating treatments and vaccines
  • A Cleveland medical center has developed a COVID-19 test that gives results in 2 hours
  • Scientists in Israel are expected to announce the development of a coronavirus vaccine
  • Meanwhile in the US, vaccination trials are already underway in Seattle
  • Distilleries across the U.S. are making their own hand sanitizers and giving it away for free
  • In South Korea, recoveries are beginning to outnumber new cases of infection
  • A Japanese flu drug has proven effective at treating COVID-19

… These are just some of the “good news” stories that are coming out every day.
That’s what we need to remember!
All over the country and the world, people are stepping up to meet this challenge.

The Link Between Your Smartphone and Joint Health

Smartphones are an essential part of the new world that we live in. They help boost productivity and enable us to stay connected. On the flip side, excessive use of smartphones can cause a variety of problems. Neck pain, back pain, and tenderness in the joints of the hands can at times, be attributed to excessive use of smartphones. In fact, 'text neck' is a phrase that's used to describe headaches, shoulder pain and arm pain due to excessive text messaging.
Individuals who spend hours slouched on the couch using their phones for texting, emailing, or even gaming tend to experience the most discomfort. This can also lead to constant pain in the thumbs and wrists due to inflammation of the tendons. This is a condition called De Quervains disease. Physical therapy offers a number of treatment options for patients with this type of joint pain and inflammation.
Pain in the Neck?
Although the impact of smartphones seems trivial, it is not something that should be taken lightly since it can result in pain, discomfort and lost productivity. Physical therapists not only help with treatment, but also with prevention of smartphone induced neck and back pain.

Your therapist can offer a variety of treatments to help patients with 'smartphone induced' symptoms, allowing them to experience pain relief and regain normal function. Medical treatment helps to reduce pain and inflammation, but physical therapy plays a vital role in the restoration of joint mobility and muscle strength.
Physical therapy treatments include:
  • Splinting – Offers rest to the joints in the hands and restricts active movement, which facilitates healing.
  • Hot/cold packs – Increase blood flow and reduce inflammation as needed.
  • Muscle stretching - Muscles in the neck and hands can become tight, which results in movement restriction. Stretching exercises can help reduce pain and inflammation while increasing range of motion.
  • Resistive exercises - An increase in muscle strength helps reduce pain and restore the ability to complete normal daily activities.
  • Manual therapy – Soft tissue massage (to target deep muscles in the neck and shoulder region) and joint mobilizations are specialized treatments by physical therapists. These treatments have a significant impact on pain relief and range of motion.
The physical therapist will also determine which posture is causing the issue that needs to be addressed. For example, if you spend several hours cradling the phone between your head and neck, your therapist will teach you important strategies like switching sides, stretching and the use of headsets. Simple things that can help save you having pain in the neck. For example, setting down the phone every 20 minutes and standing up from your desk, stretching the muscles every 30 minutes and taking a break to allow your eyes to relax are easy and powerful strategies to protect your body from the impact of smartphones.

Keeping Fibromyalgia at Bay thru Physical Therapy

According to NIH (National Institute of Arthritis and Musculoskeletal and Skin Diseases), fibromyalgia is an autoimmune disease that affects 5 million adults in the United States alone. It is more common in women than men and is generally diagnosed in individuals between the ages of 30 and 50. It is often characterized by chronic widespread muscular pain and tenderness, symptoms of extreme fatigue and listlessness, poor sleep quality, and memory problems.
Depression
Other symptoms of fibromyalgia include:  
  • Depression
  • Anxiety
  • Difficulty sleeping
  • Headaches and/or migraines
  • Numbness or tingling
  • Irritable bowel or bladder syndrome
  • Abdominal or pelvic pain and/or cramps
  • Temporomandibular pain (pain in the jaw)
There is no specific test that can actually confirm or diagnose fibromyalgia. As a result, it is extremely important to obtain an accurate diagnosis. A physician or healthcare provider will conduct several tests to rule out other conditions with similar symptoms. These conditions include rheumatologic disease and thyroid issues.
Management of Fibromyalgia with Exercise
Exercise
For a patient with fibromyalgia, every day is different. Symptoms can often come and go. Some days may be extremely challenging, while others are more manageable. A well designed exercise program plays an important role in the management of fibromyalgia but the fear of pain prevents individuals from exercising. This is where a licensed physical therapist can help.
​Expect the physical therapist to evaluate current functional limitations and establish personalized goals. The physical therapist will also evaluate the 'body mass index' of the client and determine if weight loss can help reduce symptoms.

Alzheimer's disease is a chronic, progressive, degenerative condition that causes patients to gradually lose their memory and, over time, other bodily functions. It is widely considered a disease that can rob individuals of their dignity and independence.
Traditional medicine has been unable to find a cure, although it is possible to slow down the rate at which the disease progresses. Exercise helps maintain quality of life, and it becomes even more important as the disease progresses.

Physical therapists are licensed professionals, trained to help patients with several types of neurological disorders including Alzheimer's. The therapist can recognize the challenges associated with different stages of the disease and can design a treatment program as an adjunct to traditional medical treatment.

1. Balance therapy

In severe cases, the loss of independence is so significant that a patient cannot bathe or use the bathroom independently. Physical therapists are able to help patients learn to manage these tasks with a combination of exercises and assistive devices.

A - an Aspirin a day can help keep your heart healthy. Ask your physician if low dose aspirin is a good idea for you.

 - know your Blood pressure numbers. If your blood pressure remains high for a period of time, you may be at risk for heart disease. Ask your physician for more information.

C - know your Cholesterol levels - your body has 'good' cholesterol and 'bad' cholesterol. The ratio between these numbers is an important determinant of heart health.

The Surprising Link Between Physical Therapy and Urinary Incontinence

Incontinence is an embarrassing condition that affects men, women and children of all ages. A common symptom is the loss of bladder control when coughing, sneezing, lifting and laughing.
​Incontinence can be temporary or persistent. Temporary loss of bladder control is related to diet, alcohol, caffeine and prescription medications. Underlying medical conditions such as urinary tract infections and chronic constipation also play a role.

Urinary Incontinence
Persistent loss of bladder control may be related to pregnancy, age-related changes in the bladder, menopause and enlarged prostate. Disorders of the brain and spinal cord like stroke and Parkinson's disease can also cause loss of bladder control.
​This has vast social implications for patients, who turn to adult diapers, medications and even surgical interventions.

Getting Back the 'Mind-Muscle' Link
Incontinence can be treated effectively with a variety of physical therapy techniques including, but not limited to:
​
Kegel Exercises – This involves controlled contractions of the pelvic floor muscles, using a hold and release pattern for a designated number of repetitions and sets. The frequency and intensity is gradually increased over time.
Mind - muscle
Clinical Pilates – These specialized exercises help strengthen the core and pelvic floor muscles.
Electrical Stimulation – The use of mild electrical currents to stimulate the pelvic floor muscles tends to mimic the 'hold and relax' pattern of Kegel exercises.
Biofeedback – A technique used to build the 'mind-body' connection between the brain and the muscles of the pelvic floor. ​
This helps patients identify, contract and control specific muscles surrounding the urinary tract.
​Your physical therapist may use a combination of techniques, and may design a home exercise program to help you achieve results as quickly as possible.

Regaining Control of your Life..
Incontinence can take an emotional toll on a patient. Social implications include feeling of guilt, shame, and depression in some cases. Family members and physical therapists must work together to support and help the patient prepare for 'accidents' by planning ahead prior to outdoor activities.

Yes! Physical Therapy Can Make You Smile!

​Did you know the face has 43 muscles? These are tiny muscles that control facial expressions like smiling, smirking and frowning. In fact, there is a condition called Bell's palsy that affects facial muscles, as a direct result of nerve damage.
The dysfunction affects a primary cranial nerve that controls facial muscles, resulting in temporary paralysis for some, but others experience lingering effects that can last several years.

Causes include brain tumor, stroke or Lyme disease. Other causes include viral infections like herpes simplex 1, chickenpox, German measles and mononucleosis. In some patients, no definitive cause can be found contributing to Bell's palsy.
Patients may experience difficulty blinking and closing the eyes, raising their eyebrows, and smiling and frowning. This can also affect taste. Individuals may also experience balance problems, tingling of the face, memory problems and weak muscles. Bell's palsy can appear as a single condition or as part of a larger neurological dysfunction.
Some patients achieve a spontaneous recovery and regain near-normal function. Patients may have lingering problems such as the inability to close one or both eyes, necessitating protection to prevent the eye(s) from drying out. Hearing loss is also common. Men and women are affected, and those with diabetes or upper respiratory ailments face additional risk.

Various physical therapy methods can be used to help patients with Bell's palsy. It’s essential to begin physical therapy as soon as possible. Options include:
Acupuncture/Dry Needling – Used to stimulate specific nerve and muscle sets to maintain facial tone, ease pain, and release stress. Reduces the potential for further neurological damage.
Electrical Stimulation – Stimulation of muscle and nerve groups to maintain tone and improve function. Reduces muscle 'wasting' or atrophy.
Electrotherapy
Facial Muscle Exercise – Mild facial exercises maintain facial tone and reduce muscle weakness. Activities are tailored to each individual. Improves coordination and maintains 'muscle memory'.
Clinical Pilates – A loss of balance and coordination can be treated with a specialized exercise program that focuses on small movements to build strength and regain balance.
Heat Therapy – Supervised application of hot packs helps circulation around nerves and muscles.
Biofeedback – Helps individuals regain movement control by identifying and isolating the pertinent muscles. Helps increase patient awareness of facial muscles.

The Female Athlete Triad is a group of interrelated conditions that affect female athletes, particularly teenage athletes. It is widely believed that an energy imbalance is the cause, combined with competitive forces. This disrupts eating patterns and body image for female athletes. Despite the fact that this is common in athletes, several aspects of the triad are seen in non-athletes as well.

The three medical conditions associated with the triad are:
  1. Disordered eating: anorexia, purging, induced vomiting
  2. Amenorrhea: adverse impact on menstrual cycles
  3. Osteoporosis: low bone mass/density
​Causes
There are times when the athlete exercises excessively while the body is experiencing an energy deficit. A reduced caloric intake combined with malnutrition leads to a pattern of disordered eating.
The pressure to 'be thin' is compounded by a society that idolizes celebrities and pop stars. This can result in compulsive dieting and exercise. For a growing teenage athlete, bone density can be compromised if there are deficiencies in protein, vitamins and calcium. For female athletes participating in figure skating, ballet and gymnastics in particular, awareness is critical.
Symptoms
Coaches, parents and guardians should be aware of the following warning signs:
  • Rapid weight loss or marked leanness
  • Obsession about weight, body image and food.
  • Shin splints that don't heal
  • Reduced participation or loss of interest in sports​
Exercise and Nutrition - A Healthy Foundation for Every Individual

  1. Prevention of compulsive dieting by working with a sports nutritionist.
  2. Increasing the strength of muscles, ligaments, bones and joints.
  3. Be in a progressive exercise program designed by a physical therapist.

Physical therapy for athletes begins with a detailed evaluation of the flexibility, strength, range of motion and athletic goals.
The physical therapist is a critical member of the healthcare team and works closely with a coach and athletic trainer. The physical therapist may use a combination of the following treatments:
  • Ultrasound to heal connective tissue (tendons and ligaments).
  • Manipulative therapy that includes stretching and massage.
  • Resistance training to increase muscle strength.
  • Cold compress for acute injuries and heat to relax muscular spasms.
  • Low-level laser use for muscle and connective tissue injuries.

  • The use of tape to support muscles and assistive devices as needed to support joints.

  1. Eat healthy meals at regular intervals. Use nutritional supplementation if necessary.
  2. Discuss menstrual irregularities (or sudden fluctuations in body weight) with your physician.
  3. Track exercise and calorie expenditure.

In fact, it can help improve everyone's fitness and energy levels. Physical therapy can certainly help with pain relief, restoration of function and improvement in strength and mobility, but there is a lot more that your therapist can do for you.

In fact, the treatment spectrum ranges from sports related injuries (as a result of overuse) and neurological conditions like a stroke to skeletal injuries like sprains, strains, and fractures.
Foundations of a Healthy Lifestyle
Your physical therapist will help you establish the foundation for a healthy lifestyle. Some foundational principles include:
Stretching
Find a way to include stretching into your daily routine. Associating it with something you normally do every day, like brushing your teeth or letting the dog out, can make it easier to remember.

You can set vibration alerts on your phone to remind you to take a break from work. Simply stand up, step away from the computer and stretch your neck and back. It doesn't have to be anything elaborate. Keeping healthy muscles and joints mobile throughout the day helps improve health and vitality.
Hydration
A well-hydrated body is a necessary foundation for physical health.  Make it easy to keep drinking throughout the day by keeping a water bottle at your desk. Remember to sip water at regular intervals. Don't wait till you are thirsty, because the thirst reflex is a delayed indication of hydration. If you wait till you are thirsty, you are already dehydrated.
Core Stabilization
Balance in the human body is almost as important as balance in life. Challenging yourself a little every day can strengthen your core muscles (abdominal region, trunk and lower back) and improve proprioception. Try standing or sitting (make sure you are within arms reach of a chair so you can hold on to something if needed), close your eyes and hold your arms out at shoulder height. Now try picking one leg up, slowly, beginning by lifting your heel and rolling forward through your toe. Try to hold your leg up for a few seconds. When you put your foot back down, do it slowly, setting down your toe, then the ball of your foot, and finally your heel. Don't forget to do the other side so you're equally balanced on both sides. Try to increase the duration of the one-legged stance each day.
